ARIZONA

Here are some helpful links to Arizona Industrial Commission of Arizona (ICA). These resource links change often so checking back every year or every few months would be recommended.

In Arizona, employers are required to report workplace injuries to their workers’ compensation insurance carrier and the Industrial Commission of Arizona (ICA)145. Workers who sustain an on-the-job injury must also report it to their employer within one year of the injury.

NEVADA

Report an Accident or Fatality
Effective October 1, 2017:
Report a Fatality, Inpatient Hospitalization, Amputation or Loss of an Eye
Employers must report to NVOSHA all workplace accidents involving an employee fatality or fatalities within 8 hours after learning of the accident.

Employers must report to NVOSHA all inpatient hospitalizations of one or more employees, amputations of a part of an employee’s body or an employee’s loss of an eye within 24 hours after learning of the hospitalization, amputation or loss of an eye.

Rhode Island does not have its own occupational health and safety regulatory program. Instead, Rhode Islanders fall under federal OSHA control. There is no state agency in charge of OSHA training and compliance in Rhode Island; Federal OSHA regulates OSHA training.
